If your initial application for disability benefits is denied, you will receive a notice in the mail. If you are still unable to work you will need to file an appeal. This appeal is called a Request for Reconsideration. You can file your appeal online at Social Security’s website, or you can get a paper form from your local Social Security office. Along with the Request for Reconsideration, you must file form SS-3441 (‘Disability Report”). This form asks for details about your health, your daily functioning, and your medical treatment. If you file the appeal online then you will be asked all of the questions on the 3441 form. If you are filing your appeal on paper then the Social Security office will give you this form. The appeal is due within 60 days of the date of the denial. You are given a 5 day grace period for the mail, but it is best to get the appeal is as soon as the denial is received.
